Sanka Samaranayake resolved AXIS2-1574.

Resolution: Fixed
If you define the <operation> .. <operation> element in the services.xml you
must specify the mep as follows[1]
I've tested the code with newly generated stub and it works fine. Hence I am
closing this issue.
