Handler is not being invoked

The attached module is an implementation of a LoggingModule which was done
orientating on the Axis User guide
(http://ws.apache.org/axis2/1_3/modules.html).
It was declared as a global module in the axis2.xml with a <module ref="..."/>.
The module class of it was invoked, but when a service has been called, the
Handler was not executed.
I also tried the SOAPmonitor module the same way, which was working.
I tried the module with the axis2 version service.
Is this a bug? Or am I configuring something wrong?
I am attaching the module to test for and the axis2.xml.
