I'm using the default axis webapp installation, so log4j-1.2.8.jar is in the
right place:
axis/WEB-INF/lib:
.   axis-ant.jar  commons-discovery.jar  jaxrpc.jar       saaj.jar
xerces-2.0.2.jar
..  axis.jar      commons-logging.jar    log4j-1.2.8.jar  wsdl4j.jar
xml-apis-2.0.2.jar

Also, the only change I've made to WEB-INF/web.xml is to remove the 'id'
attribute on <welcome-file-list> (iPlanet complains about this being
invalid).
