I think it is a big sparse file (blocks=40).

I do not see any problem here. Bacula tries to backup a 4TB file. I bet it
requires some time to finish. :)

No, it is not a loop. It think is a very big sparse file.

You should enable sparse file handling in FileSet resource definition for
this file. It should resolve (partially) your problem.
