I'd bet on the first one, operator error!

However, I discovered that the combination <option><shift>5 gives the single character 'fi' - could that have any relevance? The upside down question mark suggest it was trying to print an unknown single character.

I can't repeat the fi effect - must be operator error somewhere down the line.
