I have phone meetings twice a week (soon moving to videoconference meetings ... BLEAH), and the easiest way to take minutes is to type them up as folks are talking. For whatever reason, my phone doesn't mute, and my director hates it when she can hear typing in phone meetings, so I have to take all the minutes by hand when it's my turn to do so.
