Another solution would be to forget about the keywords field  
altogether and make your own field (e.g. "Category"). You can tell  
bibdesk to display your field in the left-hand pane rather than the  
keywords field. Then you would have the medline keywords if you  
needed them someday, but could organize your bibliography according  
to your own categories.
