I think a screenshot would help (cmd-shift-4, hover over main window, hit spacebar). Switch the lower pane of the main window to show icons, and zoom in (there's a slider that appears as you hover over the top of the icon view). Again, double-click that wrong icon to see if the wrong document opens.
