Aastra makes a CNX conference bridge - SIP,h.323 and PRI You can also do this easily enough with an Asterisk box. Just make sure it has some sort of Digium card in it for timing (ztdummy isn't quite good enough).
The bigger question is, are you looking for hardware recommendations, or someone to do it for you?
