You can use the rootCommand: property in your blackbox style file.  Use a
shellcommand that sets the background, e.g.

rootCommand: bsetbg ~/pictures/mybackground.jpg

When Blackbox starts up, it will run the command. See the Blackbox manual
pages.

(bsetbg is a tool contained within the Blackbox distribution that
automagically finds applications to set the root window background)

Currently not, because the main design goal of Blackbox has always been
minimalism.  You could look at IceWM (www.icewm.org) that has very
advanced window properties, while still being incredibly small and fast.
