Hello !
The option --with-mysql was enabled by default for PHP4 but is no longer
enabled by default with PHP5+
(see http://www.php.net/manual/en/ref.mysql.php).
OK, the book has a note rememering the user that PHP has many more
configure options in addition
to the proposed ones, but as MySQL belongs to the optional depenencies and
is usually used with PHP,
it could be useful to mention explicitely the --with-mysql as optional
configure option.
