The user guide is accessed by pressing Insert N to get to the
preferences and arrow down to the user guide.
http://webaim.org/articles/nvda/ also has a clear instructional page
about this great screenreader.
P.S. Though is open-source (free), the developers could likely use a
donation from their site as they've got some good ideas to make it even
better.
