#473: Improve basic details on ticket page and page structure

The design provided in #471 includes some interesting improvements to the
structure of the page beyond the merging of the activity feed and change
histories. This ticket is to:
* Allow the ticket summary to be the full {{{span12}}}
* Remove the "Activity" title
* A small restructure of the page for better placement of the activity
column
* Improve the basic details to match the "Ticket #N assigned to USER,
last modified" style
