The form is there but the link to the printable membership application, which
points to http://openid.net/foundation/join/OIDF_Application.pdf, is still
broken...

Wordpress got upgraded over the weekend and in so doing it turned off
several of the plugins that we use. One of them was the form plugin
which is now activated again. It looks like things are back to
normal. Please let me know if you see anything else out of place.
