New patch misses the case of a crash between 1 & 2. In the case, on reboot 
A.idx & A.idx.rloc exists. Now, if we delete ledger A, A.idx is deleted. 
A.idx.rloc is then orphaned. This is why I think it was be better to do the 
scan for rloc files at the bootup, rather than just in time. You'll need to add 
a start() method to the LedgerCache interface, but thats not a problem. 
EntryLogger and LedgerStorage already have a start method.
