I don't think the cost of maintaining the greatest lower bound would be high, 
but your observation about ledger ids not being monotonically increasing is a 
good one. I guess it could happen for example because of auto-replication.

In any case, I don't like the full gc idea. It would be nicer to rely upon a 
single mechanism instead of having to fall back to an alternative one.
