It runs in Terminal.app, just like all the other BSD stuff. The Terminal has kind of weird mouse support -- from discussions I've seen people can't seem to figure out what kind of terminal it's emulating, so it's hard to get everything to work like it would in an xterm or whatever -- but if you can get past that barrier it might work as you expect it to. If you've got OSX though, you have lots of options here. At a glance, I see sixteen Emacs related packages under the Fink package manager, and there's probably more than that under GNU-Darwin or other projects.
