In addition, scholarships were awarded to Colorado Health Center entry-level staff who are in training to advance their health care careers while continuing to help patients.   CCHN’s annual clinical quality awards honor outstanding Community Health Center staff members and partner safety-net providers who participate in CCHN’s quality improvement efforts. Honored this year were individuals and institutions in the Children’s Oral Health Outcomes Partnership, the Healthy Woman Outreach Project, the Colorado Oral Health Network Data for Clinical Quality Project, the CCHN Immunization Initiative, the Health Disparities Collaboratives, and the Health Profession’s Initiative Scholarship Program.    Children’s Oral Health Outcomes Partnership (COHOP)   Through collaboration between medical and dental clinics, the COHOP project helps children avoid the devastating consequences of oral disease. The following participating facilities hold themselves to the highest standards to meet the oral health needs of their patients.   Denver Community Health – Eastside Family Health Center Dolores County Health Association - Dove Creek Community Health Clinic High Plains Community Health Center, Lamar Inner City Health Center, Denver Metro Community Provider Network – Jeffco Clinic, Lakewood Peak Vista Community Health Center, Colorado Springs Pueblo Community Health Center – Routt Avenue, Pueblo; and Avondale Salud Family Health Centers – Ft. The Data for Clinical Quality Project was created to develop a system to measure, analyze, and share oral health outcomes to improve the oral health of patients. The project focuses on preventive services, completed treatment plans, increased numbers of young children accessing dental care, and availability of care for emergency cases. The following were recognized for their commitment to collaboration among all of Colorado’s non-profit dental clinics and support for improving the quality of care.   Dental Aid – Louisville Children’s Clinic Health District of Northern Larimer County Peak Vista Community Health Center Salud Family Health Centers – Ft. Lupton   The Healthy Woman Outreach Project   Participants in the Healthy Woman Outreach Project improve the health of patients by increasing their awareness of and access to clinical breast exams, mammograms, and diagnostic breast services. Teams identify barriers and implement quality improvement plans through data collection, team feedback, quality improvement technical assistance, and best practice exchange.
