My copy says;
Computer Braille code Reference Card.
In the first line it mentions about National Braille Press.  Sounds like
it can be obtained in both places.  I do not know if NBP just deals with
the US.

Hi Terry and List:

The BANA Provisional Computer Braille Code is available as a
one-volume Braille magazine-style book from the American Printing
House for the Blind.

I'm not familiar with the NBP publication, it may be on T-Braille
which was originally developed by Triformations, now Enabling
Technologies, in Florida.

If you have a QT keyboard and want to figure out the Braille
representation of a character, just type it with your reading
grade set to Computer Braille.
