The USS San Francisco, SSN 711, hit an uncharted sea mount while moving at flank speed on the way to Australia from Guam. One crewmember died as a result of the accident, he was thrown headlong onto a pump, and there were many injuries but the fact that they were able to limp back to Guam on the surface is amazing.
