Are you trying to use brltty for speech only, or do you also have a braille
device connected?

For speech, you should at least ensure that the Autospeak setting is turned on.
Then, if you'd like feedback as you type, there are several other settings for
what kind of feedback you'd like (new characters, deleted characters, replaced
characters, moving to a different character, moving to a different line,
completed words). Then there are other options for what to say when on
whitespace, and how to highlight an uppercase letter.
