Perhaps the last time it went smoothly because you used direct mode (-d).

(one way to check this quickly could be to select a few random places on both disks and compare them, but maybe some other software already does that?).
Ddrescue can't currently make such checks directly, but I plan to implement some kind of verification pass. The idea of reading from random places seems a good one.
