Having to press return twice is about the worst idea suggested here. How do you press return twice from inside a script? At least using an option, or even a funny character, instead of the mapfile is scriptable.

I try to never make a decision because "it is my tool", but because I think it is the best decision overall.
The added text in the --help output certainly helps. But maybe you could add at least a line like "warning: No mapfile specified!" to ddrescue's normal output.
I have just added the text also to the Chapter "Invoking ddrescue". I'll try to add the warning (probably at verbosity levels 1 and above) to the next version.
