It didn't take me long to think of an exception.
$ echo '.if r rst .tm rst defined' | groff
rst defined
rst (and its ilk) aren't even documented as predefined registers; the manual
says "After each use, '\w' sets several registers," which strongly implies
that before any \w escape, this set of registers is undefined. And they are,
conceptually--but not in practice.
