Don't know for sure, but it appears to have nothing to do with traps.

$ cat ab
Output.
.br
.ab Die.
$ groff -a ab
<beginning of page>
Output.
Die.
$ heirloom-troff -a ab
Die.

My hypothesis is that the other roffs are less stringent about flushing stdout
upon seeing an .ab.  You can even see that in groff:

$ fgrep -v .br ab | groff -a
<beginning of page>
Die.
