Safer mode seems to no longer be the default. This is a change between groff
1.23 ("groff" below) and git HEAD ("groff-latest" below).
$ echo '.sy true' | groff
troff:<standard input>:1: error: 'sy' request is not allowed in safer mode
$ echo '.sy true' | groff-latest
$ echo '.sy true' | groff-latest -S
troff:<standard input>:1: error: system command execution request is not
allowed in safer mode
I don't think this was an intentional change; it is not in NEWS, and the
manual still says "Safer mode is enabled by default."
