I can't reproduce this problem.  I modified your MWE only slightly to operate
in my _groff_ working copy (but still used the _groff_ binaries in
$HOME/groff-HEAD, built from e91667c1e37a93d6b44e7b1c1e4b94b01df45e41).

Incidentally is it not necessary to explicitly load "refer-ms" (nor
"refer-mm").  The full-service packages take care of this for you.  (If they
don't, that's a bug.)
