Where is the menu? What name for the menu is hardcoded in stage2? Look in
the first bytes of it if you don't know.
Did you use "d" option while installing the loader?
Does the menu appear when you boot the floppy?

Maybe you should try mkisofs for creating the CD image?
It has an option for adding boot images.
I don't know anything about Easy CD Creator 3.0
Did you say that the image is a bootable floppy image?
If I understand correctly, the options for your image might have been:
1) Bootable floopy disk image
2) Bootable hard disk image
3) Image of the whole CD (or of the first track)
Only the first option should work.
Can you mount the CD? Does it have iso9660 filesystem?
