When you boot with Grub no OS is seen so every information that Grub has is told to it by the BIOS. So if your bios likes to name your theorically second hard disk as your 4th hard disk Grub will see it as hd3.

So when Grub boots (not when using it from a shell inside Linux) device.map is ignored.
