OK, some more info. I also had a HOME environment variable pointing to
C:\HOME. This variable has been set always, but the .font.cache-1 file
in it was dated October 10, so after the one written in the user
profile. I just installed 2.7.19 and removing the cache file in
C:\HOME now recreated the file AND Lilypond runs with normal speed
again, so I'm happy.

If you want me to do some more investigating I can do that, but it
seems that 2.7.19 is good IF you make sure any .font.cache-1 files are
removed and recreated. I guess it will be stored in the user profile
if a HOME or USERPROFILE environment variable is not set.
