You are correct, Lilypond makes no attempt to follow segnos and codas. I know of no good way to repeat these structures in MIDI, but the MIDI is mostly for proofreading the entry of notes. (I tried making an additional parallel voice that spells out the "d.s. al coda" as an explicit \repeat\alternative.
