I tried your patch and built my small 'sample.texi' document again
(the same which I submitted together with my bug report).
But unfortunately it does not work :(
The resulting PDF code looks correct at first sight:

But the link does not work (tested with Acroread 4.0/WinNT and
Acroread 3.0/Solaris).
The link name appears again at several other places, the follwoing one
looks suspicious for me (though I'm not an PDF expert):

Any other ideas?
