You don't say what version of texinfo.tex you were using; the bugs sound
familiar.  Could you try the latest, from ftp://ftp.gnu.org/gnu/texinfo.tex?

texi2html is maintained separately (not by me); I think the newest
version has fixed the @image bug, among many other things.  See
