Regarding the second issue, the problem seems to be that the starred
variant (\chapter*) is used for @unnumbered:
\newcommand{\Texinfounnumberedchapter}[1]{\chapter*{#1}
\addcontentsline{toc}{chapter}{\protect\textbf{#1}}%
\renewcommand{\Texinfothechapterheading}{\Texinfoplaceholder}%
The * form shows title on a fresh line, in boldface. But it does not show
the chapter number, does not increment the chapter counter, produces no
table of contents entry, and does not affect the running header. (If you
use the page style headings in a two-sided document then the header will
be from the prior chapter.)

I found using the \chaptermark command gave better results, although
it wasn't perfect (the heading line was sometimes blank):

\chaptermark{#1}%
\renewcommand{\Texinfothechapterheading}{\Texinfoplaceholder}%
However, the documentation of \chaptermark in titleps/titlesec is very
rudimentary and I'm not sure if it's the right command to use.
