  Many directives are not allowed in .htacess files [or however you've named
  the files with the AccessFileName directive.]

  Please submit this question with a whole lot more detail to the appropriate
  users forum, such as the users mailing list or one of the newsgroups.  When
  and if you have a clear test case to reproduce the problem, please reopen
  the report with details.

  The database is not for reporting or tracking user config errors, which
  I suspect this is since I've never observed the problem you mention.
