Can't specify filter order when combining AddOutputFilter and SetOutputFilter

My mistake Joshua, you're correct.

Another fix would be to allow one to negate extentions.  Something like this:
  AddOutputFilter htmlparser !shtml

While that would fix this situation, I don't know enough about the Apache style
to know if that's an elegant solution or an ugly hack.  In any event, I think
your proposal is better.
