AddIcon for pdf and update documentation
The module autoindex uses icons which are stored in docs/icons/. There are
pdf.{gif,png} files but they are not used for .pdf files.
The patch:
- replaces the use of the generic icon to the pdf one.
- updates README files to fit the change.
