#2340: dos format shell command documentation broken

Has some obvious and serious mistakes. The example command usage is for
mkdir. I think the command is spelled incorrectly in the title "mldos".
And I think the index entry is for pwd if my memory serves correctly.
There may be more that is not right. Please review and correct.
