Deductible, blood deductible, and co-insurance are 835 adjustment reason codes; Remark 
codes are in the MIA and MOA segments, MAO09 is the Nonpayable Professional Component 
Amount - will that do?, it looks like most of what you need is there.

Tips for finding obscure elements: A word search is probably the easiest way to find 
obscure data elements; also, always search the claims adjustment reason code list - 
it's hard to find anything in there but word searches work pretty well most of the 
time.
