Tools.Surveys: Test Survey: opens a blank page

Bug description:
Steps (need demo data):
* go in 'Tools' application
* in menu, choose Surveys -> Define Surveys -> Surveys
* choose any survey [1]
* in form: click on 'Test Survey'
-> opens a blank page
PS: if you do it twice on the same survey, you can get a 'Write Concurrency'
Box but that's probably another issue
[1] I tested 'Employee Opinion', 'Job Survey' and 'OpenERP Partner Feedback'
