I use <a href="http://www.1and1.com/?k_id=6447276>1and1.com</a> hosting for all of my business and personal sites. Has phpMyAdmin built in to the control panel and php4/5 support. I had a few issues with rewriting at first, but so far has been great. Good luck on finding a great host.
