Question for you: Is your entire CakePHP application inside your web root, or just the app/webroot folder?
Ideally your webroot should be pointing to CakePHP's app/webroot folder If that is the case, then the main .htaccess file wouldn't even get called because the .htaccess file from app/webroot/.htaccess is the real one--the others just forward requests to the webroot. I'm still testing a proper solution for you, however, and will post it ASAP.
