Your Consult model would use $belongsTo.

ex.

/**
 * belongsTo associations
 *
 * @var array
 */
public $belongsTo = array(
'Patient' => array(
'className' => 'Patient',
'foreignKey' => 'patient_id',
),
);// End $belongsTo
