My thinking on this issue is that it's totally time-related.  If you
have a very short window, use something off-the-shelf so you get it
done.  If you have more time and feel the client is expecting a more
customized application, I think you either build from scratch OR use
an off-the-shelf product that you have experience with customizing.

Most of my consulting gigs have short windows, so I try to use
off-the-shelf stuff that I am confident I can customize if the demand
arises.
