I would tackle it by extending the Paginator class to include a customNumbers() style function which would take this into account. As far as I know this isn't available with the current paginator class, although I could be wrong.
A quick check on http://api.cakephp.org/class_paginator_helper.html#bd8d81f8fffdce0f12c61aa453073d99 would lead me to try wrapping an <option></option> into the before and after options to see what the output would be like, this might work.
