Usually this is the case where the RECEIVING party MUST send a reinvite.
if the receiving party doesn't send a reinvite, usually it means it 
doesn't support T38.
Not a CW problem.
This is clearly stated in the faxing page on the wiki. However, try 
T38TXDETECT or T38CALL vars (if i'm not wrong, T38TXDETECT is outdated 
and is not supported anymore).

Everything is OK.
Please remember that CW needs to do DTMF detection on the audio.
So use ulaw/alaw (it's ok, here) and INBAND dtmf mode.

Otherwise, use faxdetect() and sipt38switchover app.

If you're using inband, then please provide us with full logs and pcap 
dumps (that howto says how to produce them )
