No, this isn't currently possible.  If you use contextual menus often, maybe
you should consider purchasing a multi-button mouse. :)

b) add items to the contextual menu? Specifically, I want to add "Open

This item already exists, though it's not immediately obvious - holding down
shift will reverse the default Foreground/Background behavior in most places
in the app.  However, there's no sanctioned way to customize contextual
menus - we'd rather just get it right. ;)
