ready made ? I hate to say it, but the instrumentation industry has these things all over the place. the last time I looked one would go for about $250.00 ! As Les said, an op-amp will work well. Pretty simple to do if you want to do it yourself. If you money is no object, but speed of delivery is, google signal conditoners.
