There is (or was) also an issue with software which doesn't display file 
extensions by default, so you get malicious stuff called e.g. 
nicepicture.jpg.exe which shows up as nicepicture.jpg and people who have 
seen such as the above think they are safe to open it. As a nice bit of 
subtlety I would imagine the offending .exe file could even be made to 
display the appropriate picture as well as do its dirty work leaving the 
victim unaware of any problem.
