My favourite example, though not I think in need of maintenance, 
is the embankment at Ansty. You know that grass one that drops 
15-20 feet from the canal down to a housing estate. If that burst 
the entire canal between Sutton Stop and Hillmorton could empty
into the estate. This would be an example that probably is 
subject to regular checking, however, it does offer a vivid
example of what could go wrong if it wasn't subject to regular
preventative maintenance checks (or some freak weather and an
accident).
